Referral Procedures

Our schedulers are available Monday through Friday 8am-5pm. To schedule an appointment for your patient please call (419) 524-8151 or 1-800-686-3966. We will need the patient's name, address, phone number, pertinent medical information, date of birth and insurance information. You may print our patient information & history forms for the patient to complete.


It is our practice to forward a letter to the referring physician following any treatment or study. If you have not received a report from us after your patient's visit please contact us.

    Providing service to you and your patients is one of our primary goals. We hope that this site will better acquaint you with the resources at Mid-Ohio Heart Clinic. Since opening our practice in 1988 we have grown in many ways, but our focus has remained the same, to meet the needs of our referring physicians and our patients.

    We are affiliated with MedCentral Mansfield, Ashland Samaritan Regional Health System, SCCI, Galion and Bucyrus Hospitals. We also have satellite offices in Ashland, Galion, Bucyrus and Mt. Gilead by appointment only.
