CardiocareSimply knowing your total cholesterol is not enough. Your "good" cholesterol and "bad" cholesterol are important values to consider when determining your cardiac risk. Besides cholesterol, there are other risk factors such as smoking and high blood pressure, which can affect your heart's health.

Mid-Ohio Heart Clinic's Cardiocare program was created to help you make the most of your heart health. Cardiocare is designed to evaluate and manage your cardiovascular risk factors.

At Cardiocare, we use the National Cholesterol Education Panel Guidelines which consider your individual risk factors in order to determine a cholesterol goal that is right for you.  We can also use advanced cholesterol testing to assess lipoproteins, and concentration and size of various particles.

In addition to cholesterol management, we also focus our attention on other cardiac risk factors including high sensitivity c-reactive protein, lipoprotein (a), and homocysteine. We will work with you to develop an individualized treatment plan to guide you in making lifestyle changes to lower your risk of a heart attack.

Your primary care physician will be notified of your visit and progress in the Cardiocare program.

  

                  Initial consultation includes:
  • Reviewing your medical and family history and screening for other conditions that cause high cholesterol or high triglycerides.
  • Discussing your blood test results with a nurse practitioner who will assist you in establishing individualized goals based on national guidelines
  • Developing a treatment plan that consists of disease education, dietary instruction, an exercise plan, weight reduction, smoking cessation, and medical therapy if needed.
    Return visits include:
  • Evaluating your blood work and progress toward established goals.
  • Assessing any current symptoms related to your heart.
  • Revising the treatment plan as needed.
  • Providing continued education, support, and referrals as needed.

National Cholesterol Education Panel Guidelines

    Positive Risk Factors
  • Age:
    Male > 45 years
    Female > 55 years or
    premature menopause without estrogen replacement
  • Family history of premature heart disease
  • Current cigarette smoking
  • Hypertension (>140/90 mmHg) or on a medication to lower the blood pressure
  • Low HDL cholesterol (<40)
  • Diabetes mellitus
    Negative Risk Factors
  • High HDL cholesterol level (>60; subtract one risl factor)
